Certified Nursing Assistant FT (4pm -12 am)


Looking for dedicated caregiver with a valid CNA certification in NJ.

Requirements:
-Possess the knowledge and skills necessary to provide care/services to geriatric residents.
-Be able to move intermittently throughout the day; sit, stand, bend, and lift.
-Attend to the needs of residents by assisting with bathing, feeding, transporting, etc.
-Comply with all safety and infection control procedures.
-Maintain confidentiality of all pertinent resident care information to assure resident rights are protected.
-Consistently maintain a kind, caring, and respectful attitude towards residents, visitors, and staff.

FT MDS Coordinator

Seashore Gardens Living Center is looking for a Registered Nurse with experience in MDS (Minimum Data Set) and RAI (Resident Assessment Instrument).

Responsibilities include:

·Complete MDS, care plans, and assessments for all residents in an accurate and timely manner.

·Identify residents’ care needs and communicate with resident, family, peers and staff to promote quality care.

·Develop care plans and determine activities that will result in the best outcome for the resident.

·Verbally communicates to the DON/ADON the effectiveness of the care plan.

·Documenting all changes in residents’ condition.

·Provide care and services that assures confidential information and residents’ rights be protected at all times.

Requirements & Qualifications:

·Position requires a current and valid RN license in the state of New Jersey.

·Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices, procedures, regulations and guidelines governing long-term care.

·Current knowledge of MDS and RAI processes according to the state and federal guidelines.

·Must be able to make decisions and possess strong communication and leadership skills.

·Be able to organize and prioritize many tasks timely and effectively.

·Consistently works cooperatively with residents, family members, physicians and other personnel of the home.

·Must have computer skills.

·Experience in a long term care facility is preferred.
